Care Management Program Expanded to Better Serve Patients

Golden West Chapter

Serving people living with ALS in: 31 Counties throughout California and the state of Hawaii

The Ice Bucket Challenge brought new resources to The ALS Association Golden West Chapter to advance the search for the cure and improve care for people with ALS.  With this additional funding, the chapter has expanded Professional Care Management hours throughout its service area (31 counties in California and the state of Hawaii) to assist people with ALS and their families in finding solutions to their needs. Through in-home visits, emails, and phone calls, the chapter’s Professional Care Managers make assessments, troubleshoot, update treatment plans, make referrals to supportive services, and advocate with insurance companies and government agencies on behalf of people with ALS.

The chapter has also increased the number of monthly support groups in its service area.  The chapter offers more than 20 monthly gatherings featuring educational speakers and discussions to help people with ALS and their families learn strategies for living with ALS, including specialized groups for caregivers and veterans.

Finally, the Ice Bucket Challenge funds have allowed the chapter to augment support for affiliated ALS Association Certified Treatment Centers of Excellence and multidisciplinary clinics.  ALS clinic program participation helps people with ALS to live longer and often better.  Multidisciplinary clinic sites also provide people with ALS opportunities to participate in clinical trials and basic research.
